REQUEST FOR PROPERTY LISTING or UPDATE

The following form is used to request OAG Hotels At Per Diem to add your hotel to the listings on the www.hotelsatperdiem.com. Upon your completing and submitting this form, your hotel will be listed within 7-14 days. You will receive a confirmation email wihtin 1-3 days. This will give you the date your listing will appear.

You may also use this form to UPDATE your current listing. Simply submit the form with the correct information and we will send a confirmation as well.

By submitting this request to OAG Hotels At Per Diem you are:

1. agreeing to offer the existing Lodging Per Diem rate to Government travelers on Official Business; and,
2. validating that your property meets the fire and life requirements of the Hotel and Motel Fire Safety Act of 1990 by providing a Fire Safety Approval Number (FEMA Approval No.) that is assigned by the U. S. Fire Administration

Notice!!! Please do not submit the following request form unless your property has a VALID FEMA Approval Number. They will not be processed. If you do not know whether your property has a FEMA Approval Number, check the Federal Emergency Management Agency's (FEMA) U. S. Fire Administration website for verification and application information. The FEMA hotel database is on http://www.usfa.dhs.gov. Go to the link titled Hotel/Motel Fire-Safe List to search for your FEMA number. You can also update your information or request a FEMA number on this website.
